Licensed Vocational Nurse (LVN)

The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Education

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.

  1. Must have a minimum, six(6) months training and experience in rehabilitative and restorative nursing practices.
  2. Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN in California.

The primary purpose of your job position is to plan, organize, develop and direct the overall of our Nursing Service Department in accordance with current federal, state and local standards, guidelines and regulations that govern our facility and as may be directed by the administrator and the Medical Director, to ensure that the highest degree of quality care is maintained at all times.

Essential Responsibilities

  1. Maintain an acceptable standard of nursing practice and professional decorum.
  2. Be Courteous, considerate and cooperative when communicating with all Facility staff, residents and public.
  3. Promote and maintain good public relations on behalf of the facility.
  4. Knowledge of Federal and State Regulations governing long term care nursing facilities.
  5. Knowledge and implementation of the facility’s policies and procedures.
  6. Direct and assist Licensed Nurses in clinical performances areas.
  7. RN: Perform functions and responsibilities of Assistant Director of Nursing Services on the day shift as directed.
  8. Reach appropriately to emergencies and disaster situations.
  9. Notify Administrator and/or Director of Nursing Services of any serious situation requiring administrative direction.
  10. Perform other duties as may be assigned by the Director of Nursing Services.
  11. Other duties and responsibilities may be assigned by the Administrator due to facility needs.

Your Rehabilitation

At Greenfield Care Center of Fairfield have therapist on-site to help you get the treatment you deserve. Our therapist work with patients and their families to provide balanced and continual care.

Our extensive rehabilitation services emphasize achievement. Each resident’s recovery potential is assessed and goals for improvement are established. Our rehabilitation program is exclusively provided by EVA CARE INC. and consists of physical therapy, occupational therapy and speech-language pathology. Each therapist is properly trained and certified in their field of specialty.

Physical Therapy

Physical Therapy is mainly to assist residents and patients to achieve their maximum functional mobility skills. Individualized treatment programs are designed to alleviate pain, correct deformities, prevent disability, and restore functional ability to the highest level.

Here at Greenfield Care Center of Fairfield a team of licensed therapist are available to work with each resident to reduce pain and improve strength, balance, mobility and endurance.

Occupational Therapy

Occupational Therapy concerns with persons’ performance in activities necessary for daily life, and provides specialized services that enhance the abilities of individuals to perform and to achieve satisfaction in their daily occupations. Programs are individually designed to assist them to reach the highest independent level in all daily living skills.

Our occupational therapist treat conditions of the upper extremity. Residents learn how to increase their independence with everyday task such as dressing and bathing safely.

Speech-Language Pathologist

Speech-Language Pathologists conduct evaluation and implement individual treatment programs for residents and patients with speech, language, and swallowing disorders. Communication skills are enhanced and techniques to ensure safe swallowing are taught.

Rehabilitation therapy involves direct manipulation of muscles, joints and other parts of the body affected by an injury or chronic illness. Strength training, heat treatments, massage and supervised exercises may all be elements of rehabilitation therapy. Individual therapy regimens will depend on the type of injury or condition, the patient’s age and specialized treatments prescribed by a physician.

Our speech therapist help residents improve language and communication skills, and swallowing abilities.
